1. I understand that as a result of the effects generated by the infectious disease called Covid-19, which has been categorized as a pandemic by the World Health Organization, the national government has decided to apply emergency measures in order to guarantee public health in the country and prevent the spread of the virus, for which it has specially regulated the entry into the national territory of people nationals, residents and foreigners.

2. I declare that I do not suffer or suffer from any condition related to the Covid-19 virus and that, in any case, I am willing to submit to all the examinations and measures of prevention and sanitary control imposed by the Panamanian authorities, in order to avoid the spread of the above mentioned virus, before traveling to Panama. I hereby declare that I am willing to undergo all the examinations and measures of prevention and sanitary control imposed by the Panamanian authorities, in order to avoid the spread of the above mentioned virus, before traveling to Panama. I hereby bind myself to comply with the sanitary control measures and health protocols established by the national government through the Ministry of Health (MINSA) and to comply with the MANDATORY isolation and/or quarantine, in the event that the authorities of the Ministry of Health so require.

3. I declare that all the information referred to in the document to enter the Republic of Panama is true. This is a sworn declaration under artículo 308 del Código Penal de la República de Panamá. Whoever spreads a dangerous or contagious disease to persons or violates the sanitary measures adopted by the competent authorities to prevent the introduction or spread of a disease shall be punished with imprisonment of four to six years. In the case of a contagious disease, the penalty shall be ten to fifteen years imprisonment.
